1. Overview and Causes

Bacillary dysentery is an infectious disease of the intestinal tract caused by bacteria of the genus Shigella and is spread by contact with a patients or carriers or though food or water contaminated by their feces. It commonly occurs in summer and fall. Clinically, It can be divided into acute and chronic types according to the clinical features and courses.

2. Symptoms

1. Acute bacillary dysentery
a. In epidemic seasons, the history of eating contaminated food and close contact with dysentery patients is helpful to diagnosis.
b. fever, abdominal pain, diarrhea, tenesmus and bloody purulent stools are major manifestations.

2. Chronic bacillary dysentery
a. Having a past history of acute bacillary dysentery
b. Having recurrent or persistent abdominal pain, accompanied with diarrhea or alternation of diarrhea and constipation. Intermittent appearance of mucopurulent and blood-stained stools.

3. TCM Effectiveness and Evaluation

Bacillary dysentery is successfully treated by TCM’s treatment approach, specially in chronic cases. It may take about one or two months as a treatment cycle.

4. Common Syndromes and Treatments in TCM

This ailment, according to TCM, has different syndromes. The treatment for these syndromes using herbal medicine and acupuncture is shown below.

Syndrome (a)
Acute bacillary dysentery

1. Dysentery due to Dampness and Heat
Abdominal pain, diarrhea with bloody mucous stool which is moved several times with a small amount each time, tenemus, burning sensation at the anus, scanty deep- colored urine, or accompanied with chill and fever, yellow and greasy coating of the tongue, slippery and rapid pulse.

To treat by Herbal medicine

Therapeutic principle: Clearing away heat and eliminating dampness and regulating Qi to remove stagnancy.

Prescription/Formula: White peony root( Baishaoyao) 20g, Coptis rhizome (Huanglian) 10g, Scutellaria root( Huang qin)10g, Honeysuckle flower (Jinyinhua)30g, Portulaca (Machijian) 30g, Rhubarb(Dahuang)6g, Aucklandia root(Muxiang)6g, Areca root(Binlaing) 10g, Bitter orange(Zhike) 10g Licorice root(Ganzao)6g


2. Dysentery due to Heat and Toxic
Sudden onset, violent attack, purulent and bloody stools, high fever, restlessness or even coma or convulsion in serious cases, red tongue or deep-red tongue with yellow and greasy coating and rapid pulse.

To treat by Herbal medicine

Therapeutic principle: Removing heat and Toxic and cooling the Blood.

Prescription/Formula: Pulsatilla root(BaiTouwong)30g, Ash bark(Qinpi)15g, Coptis rhizome (Huanglian) 10g, Phekllodendron( Huangbai)10g, Honeysuckle flower (Jinyinhua)30g,Forsythia fruit(Lianqiao)15g, Moutan Bark(Mudanpi) 10g,Red peony root(Chishao)10g, Sanguisorba root(Diyu)15g,Pueraria root(Gegen)15g

  

To treat by Acupuncture and Moxibustion
Prescription: Hegu(LI4), Tainshu(ST25), Shangjuxu(ST37), Quchi(LI11), Neiting(ST44).

Syndrome (b)
Chronic bacillary dysentery

1. Deficiency of Qi

Chronic dysentery with frequent relapse, and slow recovery, abdominal pain and tenesmuns during the attack, dark-colored mucous stools, fatigue and tired, pale tongue with greasy coating, deep and weak pulse.

To treat by Herbal medicine

Therapeutic principle: Invigorating the Spleen, replenishing Qi, removing heat and eliminating dampness.

Prescription/Formula: (Dangshen)15g, White atractylodes rhizome (Baizhu)10g, Poria(Fuling)15g, Aucklandia root(Muxiang)10g, Tangerine peel( Chenpi)10g, Coptis rhizome (Huanglian) 10g, Pulsatilla root(BaiTouwong)20g, Sanguisorba root(Diyu)15g,Prepared Licorice root(Zhigancao)6g.

2. Deficiency of Yang

Chronic dysentery with watery and purulent stools, abdominal pain with preference for warmth and pressure, cold extremities, loss of appetite, listlessness, pale tongue with greasy coating, deep and fine pulse.

To treat by Herbal medicine

Therapeutic principle: Warming and invigorating the Spleen and kidney.

Prescription/Formula: (Dangshen)15g, White atractylodes rhizome (Baizhu)10g,Baked ginger(Paojiang)10g, Cinnamon(Rougui)6g, Nutmeg (Kezi)15g, Aucklandia root(Muxiang)6g, Red halloysite (Chishizhi)12g, Prepared Licorice root(Zhigancao)6g.

  

To treat by Acupuncture and Moxibustion

 1) Body Acupuncture
Prescription: Hegu(LI4), Tainshu(ST25), Shangjuxu(ST37),Pishu(BL20), Guanyuan(RN4).

2) Ear Acupuncture
Prescription: Pt. Large Intestine, Pt. Small Intestine, Pt. Stomach, Pt. Spleen, Pt. Sympathetic Nerve, Pt. Shenmen.

5. Healing by Other Therapies

Generally speaking, Qigong is good for healing effect of this specific ailment, however individual results may vary. It improves the flow of the qi and the circulation of the blood, so you'll experience more energy, but, you won't see/ feel any immediate effects, as with many exercises and therapies, it takes a few weeks or even months of practice before you'll see any significant improvements.

(b) Medicated diet
1. Honeysuckle Flower (Jinyinhua) 30g, Portulaca (Machijian) 30g, put 4 cups of water then soak 20 minutes, after boiling 30 minutes to make tea, drink twice a day for two weeks.
2. Hutneg (Roudoukou) 10g, Cinnamon Bark (Rougui) 5g, Tangerine peel (Chenpi) 10g, lean pork 100g, with 4 cups of water to decoct 1-2 cups of soup to drink twice a week for two months.
(c) Life style
1. Avoid mental stress and overstrain.
2. Avoid to eat raw, cold, special food and deep-fried food.
3. Take regular meals on time, avoid excessive food and/or drink at one meal.
