1. Overview and Causes

Chronic renal (kidney) failure generally occurs over a number of years as the kidney's nephrons (main functioning units of the kidney) are destroyed. This is a particularly insidious disease because often there are no symptoms in its early stages. In many cases, no symptoms are apparent until kidney function is decreased to less than 25 percent of what is considered normal. This disease is included in the categories of "shui zhong" and "xu lao" in Traditional Chinese Medicne (TCM).

2. Symptoms

(a) Abnormal urinalysis (tsts done to check the function of the kidneys)
(b) Hypertension (high blood pressure)
(c) Loss of weight, nausea, vomiting, weakness, fatigue
(d) Headache
(e) Decreased urine output
(f) Decreased mental activity
(g) Muscle twitches and cramps
(h) Gastrointestinal bleeding
(i) Yellowish brown cast to the skin, itching

4. Common Syndromes and Treatments in TCM

This ailment, according to TCM, has different syndromes. The treatment for these syndromes using herbal medicine and acupuncture is shown below.

Syndrome (a)
Deficiency of the spleen-yang and kidney-yang

Deficiency of yang in the spleen and the kidney results in decreased mental activity, a disinclination to talking, an aching pain around the waist, a weakness in the legs, an aversion to cold, cold limbs, loose stools, a pale complexion, scanty urine, nausea and vomiting. There may also be a paleness in the tongue with a thin and white fur. The pulse would be weak.

To treat by Herbal medicine
Therapeutic principle: Herbal medicine can help Invigorate the spleen and warm the kidney

Prescription/Formula: Modified decoction for tonifying the kidney-qi Dried rehmannia root (Gan Di Huang) 30g, Chinese yam (Shan Yao)15g, dowgood fruit (Shan Zhu Yu)15g, oriental water plantain (Ze Xie)10g, poria (Fu Ling )10g, moutan bark (Mu Dan Pi)10g, cinnamon twig (Gui Zhi)6g , prepared aconite root (Pao Fu Zi)6g.

Syndrome (b)
Insufficiency of the liver-yin and kidney-yin

A patient suffering from a deficiency in liver and kidney yin may suffer from vertigo (false sensation of imbalance and of the movement of surroundings), tinnitus (ringing in the ears), deafness, nausea, vomiting, weakness of the waist and knees, and a warm and feverish sensation in the palm and the soles. He or she may also experience a urine taste, or a dry bitter taste in the mouth. The tongue is red with little fur, while the pulse is rapid.

Therapeutic principle: Herbal medicine can nourish the kidney and enrich the liver and kidney

Prescription/Formula: Modified decoction of six drugs including rehmannia Prepared rhizome of rehmannia (Shou Di) 30g, dogwood fruit (Shan Yu Rou)15g, dried Chinese yam (Gan Shan Yao)15g, oriental water plantain (Ze Xie)10g, poria (Fu Ling) 10g, moutan bark (Mu Dan Pi)10g, glossy privet fruit (Nu Zhen Zi)12g, eclipta (Han Lian Cao)12g.

Syndrome (c)
Retention of water-dampness within the body

A person with retention of water-dampness, and edema (swelling) of body, pleural effusion (accumulation of fluid in the lung spaces), ascites (accumulation of fluid in the abdomen), chest stuffiness, shortness of breath, abdominal distension, nausea, vomiting, urine taste in mouth, scanty urine or no urine, plump tongue, deep and taut pulse.

To treat by Herbal medicine
Therapeutic principle: Excreting dampness and water.

Prescription/Formula: Modified decoction of tetrandra, pricklyash seed, lepidium seed and rhubarb. Tetrandra root (Fang Ji) 10g, pricklyash seed (Jiao Mu)6g, lepidium seed (Ting Li Zi)10g, rhubarb rhizome (Da Huang)6g.

Syndrome (d)
Blocking of channels and collaterals by blood stasis

Darkish complexion, dark-purple lips or dark eyes, deep-red tongue with ecchymoses (purple mottling of skin), and uneven pulse.

To treat by Herbal medicine
Therapeutic principle: Promoting blood circulation to remove blood stasis.

Prescription/Formula: Modified decoction of reinforcing middle-warmer and replenishing qi and modified decoction of cinnamon twig and poria Astragalus root (Huang Qi)15g, prepared licorice root (Gan Cao)6g, ginseng (Ren Shen)10g, Chinese angelica (Dang Gui) 10g, cimicifuga (Sheng Ma )3g, bupleurum root (Chai Hu)3g, cinnamon twig (Gui Zhi) 10g, poria (Fu Ling)10g, red peony root (Chi Shao)9g, peach kernel (Tao Ren) 9g.

Syndrome (e)
Stirring of liver-wind inside body

Stiff neck and numb limbs, trembling of hands and feet or contraction of hands and feet, dizziness, headache, lethargy or unconsciousness, red tongue with little fur, thready pulse.

To treat by Herbal medicine
Therapeutic principle: Calming the liver to stop wind produced inside the body.

Prescription/Formula: Tranquilizing liver-wind decoction Achyranthes root (Niu Xi) 30g, red ochre (Dai Zhe Shi)30g (ground into fine powder), dragon's bone ( Long Gu)15g, oyster shell (Mu Li)15g, tortoise plastron (Gui Ban)15g, white peony root (ShaoYao) 15g, scrophularia root (Xuan Shen)15g, lucid asparagus root (Tian Dong) 15g, Sichuan chinaberry (Chuan Lian Zi)6g, germinated barley (Sheng Mai ya)6g, oriental wormwood (Yin Chen)6g, licorice root (Gan Cao)4g.

5. Healing by Other Therapies

Generally speaking, Qigong is good for healing effect of this specific ailment, however individual results may vary. It improves the flow of the qi and the circulation of the blood, so you'll experience more energy, but, you won't see/ feel any immediate effects, as with many exercises and therapies, it takes a few weeks or even months of practice before you'll see any significant improvements.

(b) Medicated diet
1.Crucian carp (weighs about 250g), Garlic 10g
Cut open the belly of Crucian carp, remove the internal organs, but keep the scales. After cleaning it, put the garlic into the belly of the fish, wrap the fish with clean and white paper, soak it with water, then put it into rice husk to bake it until it is done. Eat all the garlic. Eat one Crucian carp a day.
2. Take low-protein food with high nutrition.
(c) Life style
1. Scratching should be avoided; during acute onset, the affected part should not be bathed in hot water.
2. Refrain from eating seafood, eggs, beef, mutton, chicken and pungent food.
3. Don't take any alcohol and soft drinks.
4. Take good rest, and take precautions against infection.
